Referring a patient

You should not need a form to start a conversation.

1. Call the clinical line

2. We verify coverage

Usually the same day, and we tell you before anyone is promised anything.

3. First visit within 24 hours

Sooner if symptoms are not controlled. Evenings and weekends included.
